Introducing ALW Care Coordination

CENTRAL COAST HOME HEALTH specializes in orchestrating comprehensive services within your home health network.

Enrollment Process

Apply-Pre-Eligibility

Please ensure that you thoroughly fill out and submit the application packet to ascertain your compliance with the pre-eligibility criteria. If you meet all the specified pre-eligibility criteria, Central Coast Healthcare Services will provide support in facilitating your enrollment into the ALW program, and this assistance will be provided at no cost to you.

Waitlist Process

If your Currently residing in SNF/ Hospital for over 60 days, you may be eligible for an immediate assessment, and a Waitlist Request is not necessary. If you are currently living at home or in an assisted living environment, you will be added to the DHCS Waitlist upon submission of your application. After receiving your application, we will process your details and forward a Waitlist Request to DHCS. Please note that the Central Coast ALW Waitlist typically has an average waiting period of 6-12 months.* Once your waitlisting is released, our team will reach out to you to arrange the Initial Assessment.

Assessment

An RN Care Coordinator from our team will perform an in-person or virtual assessment to assess your Medical ALW Eligibility using the DHCS Assessment Tool. This tool will comprehensively evaluate and review all your care requirements. If deemed eligible, our team will thoroughly review the assessment and submit the complete documentation to DHCS for approval.

Review & Approval

The Department of Health Care Services may require up to 45 days to complete the assessment approval process. Upon the conclusion of the review and receipt of approval, there is a 180-day window, starting from the initial assessment submission date, within which we must secure placement at an ALW Approved Facility.

Transition Into An ALW Approved Facility

With a selection of over 800 ALW Approved Facilities in Southern California, our team is dedicated to collaborating with you and your loved ones to ensure the placement at an ALW Approved Facility that aligns with your specific care requirements.

Ongoing Care Oversight

In our role as the Care Coordination Agency (CCA), we remain committed to actively monitoring and overseeing participants' care throughout their enrollment. An RN Care Coordinator or a social worker will conduct monthly visits to check in, ensuring that participants are receiving the necessary and appropriate care at the facility.
